Community transition services are designed to help individuals with intellectual and developmental disabilities move from nursing homes, institutions, hospitals, or other care facilities back into the community setting.
Transitioning from institutional care to community-based living is a significant step towards independence and integration for individuals with disabilities or chronic illnesses.
Our community transition services help facilitate this move by providing financial assistance for essential needs. This includes securing accessible and appropriate housing options in the community and paying for necessary and basic household items like furniture, kitchen supplies, and bedding.
Transitioning from institutional care to the community involves careful planning and coordination among various stakeholders, including healthcare providers, social workers, family members, and community organizations.
We plan out each transition tailored to meet the specific needs and preferences of the individual with disability. To ensure a smooth transition and utmost support, we provide necessary training on managing personal finances, navigating community resources, and accessing healthcare. We also follow up to see if our clients have adjusted to their new living environment, are connecting with community resources, and address any emerging needs.
Services under the Community Integration and Habilitation (CIH) waiver allow people to stay in their homes or other community-based settings. The CIH waiver also helps people move from other institutions or state-run facilities into individual community settings.
