Noble Abode Care has developed a personalized Wellness Coordination Plan for each of our clients. This plan addresses routine and non-routine healthcare needs, ensuring a holistic approach to individuals with disabilities for their health and well-being.

We offer Wellness Coordination for individuals in our Supported Living program. Through this program, we ensure that caregivers are well-informed and equipped to manage and support the health requirements of the individuals in their care.

We arrange appointments, follow up on treatments, and ensure that all medical advice is thoroughly followed. NAC’s Wellness Coordination service aims to provide a structured and proactive approach to the healthcare system by coordinating with nurses, doctors, and other healthcare professionals. This means we go beyond the routine doctor visits.

Our wellness coordination services are available under the FSW and CIH waiver.
